class=”ssrcss-uf6wea-RichTextComponentWrapper e1xue1i83″&gt;&lt;div class=”ssrcss-3z08n3-RichTextContainer e5tfeyi2″&gt;&lt;p&gt;About 1.6 million twins are born each year worldwide, with one in every 42 children born a twin.&lt;/p&gt;&lt;/div&gt;&lt;/div&gt;&lt;div data-component=”text-block” class=”ssrcss-uf6wea-RichTextComponentWrapper e1xue1i83″&gt;&lt;div class=”ssrcss-3z08n3-RichTextContainer e5tfeyi2″&gt;&lt;p&gt;Delayed childbearing and medical techniques such as IVF have seen the rate of twin births rise by a third since the 1980s.&lt;/p&gt;&lt;/div&gt;&lt;/div&gt;&lt;div data-component=”text-block” class=”ssrcss-uf6wea-RichTextComponentWrapper e1xue1i83″&gt;&lt;div class=”ssrcss-3z08n3-RichTextContainer e5tfeyi2″&gt;&lt;p&gt;But it could be all downhill from here as the focus shifts to one baby per pregnancy, which is less risky.&lt;/p&gt;&lt;/div&gt;&lt;/div&gt;&lt;div data-component=”text-block” class=”ssrcss-uf6wea-RichTextComponentWrapper e1xue1i83″&gt;&lt;div class=”ssrcss-3z08n3-RichTextContainer e5tfeyi2″&gt;&lt;p&gt;According to a global overview in &lt;a href=”https://academic.oup.com/humrep/article-lookup/doi/10.1093/humrep/deab029″ class=”ssrcss-hiczm3-InlineLink e1no5rhv0″&gt;the journal Human Reproduction&lt;/a&gt;, the peak was reached because of large increases in twinning rates in all regions over 30 years – from a 32% rise in Asia to a 71% rise in North America.&lt;/p&gt;&lt;/div&gt;&lt;/div&gt;&lt;div data-component=”text-block” class=”ssrcss-uf6wea-RichTextComponentWrapper e1xue1i83″&gt;&lt;div class=”ssrcss-3z08n3-RichTextContainer e5tfeyi2″&gt;&lt;p&gt;The researchers collected information on twinning rates from 165 high in Europe and North America – and worldwide it’s gone from nine per 1,000 deliveries to 12.&lt;/p&gt;&lt;/div&gt;&lt;/div&gt;&lt;div data-component=”text-block” class=”ssrcss-uf6wea-RichTextComponentWrapper e1xue1i83″&gt;&lt;div class=”ssrcss-3z08n3-RichTextContainer e5tfeyi2″&gt;&lt;p&gt;But twin rates in Africa have always been high and haven’t changed much over the past 30 years, which could be due to population growth.&lt;/p&gt;&lt;/div&gt;&lt;/div&gt;&lt;div data-component=”crosshead-block” class=”ssrcss-mysbf6-ComponentWrapper-CrossheadComponentWrapper e1xue1i84″&gt;&lt;h2 class=”ssrcss-qozapo-StyledHeading e1fj1fc10″&gt;Helping hand&lt;/h2&gt;&lt;/div&gt;&lt;div data-component=”text-block” class=”ssrcss-uf6wea-RichTextComponentWrapper e1xue1i83″&gt;&lt;div class=”ssrcss-3z08n3-RichTextContainer e5tfeyi2″&gt;&lt;p&gt;Africa and Asia make up about 80% of all twin deliveries in the world at present.&lt;/p&gt;&lt;/div&gt;&lt;/div&gt;&lt;div data-component=”text-block” class=”ssrcss-uf6wea-RichTextComponentWrapper e1xue1i83″&gt;&lt;div class=”ssrcss-3z08n3-RichTextContainer e5tfeyi2″&gt;&lt;p&gt;Prof Christiaan Monden, the study’s author from the University of Oxford, said there was a reason for that.&lt;/p&gt;&lt;/div&gt;&lt;/div&gt;&lt;div data-component=”text-block” class=”ssrcss-uf6wea-RichTextComponentWrapper e1xue1i83″&gt;&lt;div class=”ssrcss-3z08n3-RichTextContainer e5tfeyi2″&gt;&lt;p&gt;”The twinning rate in Africa is so high because of the high number of dizygotic twins – twins born from two separate eggs – born there,” he said.&lt;/p&gt;&lt;/div&gt;&lt;/div&gt;&lt;div data-component=”text-block” class=”ssrcss-uf6wea-RichTextComponentWrapper e1xue1i83″&gt;&lt;div class=”ssrcss-3z08n3-RichTextContainer e5tfeyi2″&gt;&lt;p&gt;”This is most likely to be due to genetic differences between the African population and other populations.”&lt;/p&gt;&lt;/div&gt;&lt;/div&gt;&lt;div data-component=”text-block” class=”ssrcss-uf6wea-RichTextComponentWrapper e1xue1i83″&gt;&lt;div class=”ssrcss-3z08n3-RichTextContainer e5tfeyi2″&gt;&lt;p&gt;Twinning rates in Europe, North America and Oceanic countries have been catching up – and the increasing use of medically assisted reproduction since the 1970s – for example IVF, ICSI, artificial insemination and ovarian stimulation – have been the main reason.&lt;/p&gt;&lt;/div&gt;&lt;/div&gt;&lt;div data-component=”image-block” class=”ssrcss-18mjolk-ComponentWrapper e1xue1i80″&gt;&lt;figure class=”ssrcss-2y05cd-StyledFigure e34k3c20″&gt;&lt;div likelihood of a multiple birth.&lt;/p&gt;&lt;/div&gt;&lt;/div&gt;&lt;div data-component=”text-block” class=”ssrcss-uf6wea-RichTextComponentWrapper e1xue1i83″&gt;&lt;div class=”ssrcss-3z08n3-RichTextContainer e5tfeyi2″&gt;&lt;p&gt;Women choosing to start families later in life, increased use of contraception and lower fertility overall also play a role, the review says.&lt;/p&gt;&lt;/div&gt;&lt;/div&gt;&lt;div data-component=”text-block” class=”ssrcss-uf6wea-RichTextComponentWrapper e1xue1i83″&gt;&lt;div class=”ssrcss-3z08n3-RichTextContainer e5tfeyi2″&gt;&lt;p&gt;But the emphasis is now on singleton pregnancies, which are safer, says Prof Monden.&lt;/p&gt;&lt;/div&gt;&lt;/div&gt;&lt;div data-component=”text-block” class=”ssrcss-uf6wea-RichTextComponentWrapper e1xue1i83″&gt;&lt;div class=”ssrcss-3z08n3-RichTextContainer e5tfeyi2″&gt;&lt;p&gt;”This is important as twin deliveries are associated with higher death rates among babies and children, and more complications for mothers and children during pregnancy, and during and after delivery,” he says. &lt;/p&gt;&lt;/div&gt;&lt;/div&gt;&lt;div data-component=”text-block” class=”ssrcss-uf6wea-RichTextComponentWrapper e1xue1i83″&gt;&lt;div class=”ssrcss-3z08n3-RichTextContainer e5tfeyi2″&gt;&lt;p&gt;Twins have more complications at birth, are more often born premature and have lower birth weights and higher still birth rates.&lt;/p&gt;&lt;/div&gt;&lt;/div&gt;&lt;div data-component=”crosshead-block” class=”ssrcss-mysbf6-ComponentWrapper-CrossheadComponentWrapper e1xue1i84″&gt;&lt;h2 class=”ssrcss-qozapo-StyledHeading e1fj1fc10″&gt;Survival chances&lt;/h2&gt;&lt;/div&gt;&lt;div data-component=”text-block” class=”ssrcss-uf6wea-RichTextComponentWrapper e1xue1i83″&gt;&lt;div class=”ssrcss-3z08n3-RichTextContainer e5tfeyi2″&gt;&lt;p&gt;The review found that the fate of twins in low and middle-income countries was more of a concern.&lt;/p&gt;&lt;/div&gt;&lt;/div&gt;&lt;div data-component=”text-block” class=”ssrcss-uf6wea-RichTextComponentWrapper e1xue1i83″&gt;&lt;div class=”ssrcss-3z08n3-RichTextContainer e5tfeyi2″&gt;&lt;p&gt;In sub-Saharan Africa, in particular, many twins will lose their co-twin in their first year of life – more than 200,000 each year.&lt;/p&gt;&lt;/div&gt;&lt;/div&gt;&lt;div data-component=”text-block” class=”ssrcss-uf6wea-RichTextComponentWrapper e1xue1i83″&gt;&lt;div class=”ssrcss-3z08n3-RichTextContainer e5tfeyi2″&gt;&lt;p&gt;”While twinning rates in many rich Western countries are now getting close to those in sub-Saharan Africa, there is a huge difference in the survival chances,” said Prof Jeroen Smits, a study author.&lt;/p&gt;&lt;/div&gt;&lt;/div&gt;&lt;div data-component=”text-block” class=”ssrcss-uf6wea-RichTextComponentWrapper e1xue1i83″&gt;&lt;div class=”ssrcss-3z08n3-RichTextContainer e5tfeyi2″&gt;&lt;p&gt;Looking ahead, the researchers say India and China will play a major role in future twinning rates.&lt;/p&gt;&lt;/div&gt;&lt;/div&gt;&lt;div data-component=”text-block” More than 23 million people have had a first dose of coronavirus vaccine.&lt;/p&gt;&lt;/div&gt;&lt;/div&gt;&lt;div data-component=”crosshead-block” class=”ssrcss-mysbf6-ComponentWrapper-CrossheadComponentWrapper e1xue1i84″&gt;&lt;h2 class=”ssrcss-qozapo-StyledHeading e1fj1fc10″&gt;Infections in the community&lt;/h2&gt;&lt;/div&gt;&lt;div data-component=”text-block” class=”ssrcss-uf6wea-RichTextComponentWrapper e1xue1i83″&gt;&lt;div class=”ssrcss-3z08n3-RichTextContainer e5tfeyi2″&gt;&lt;p&gt;According to the ONS statistics, there are around 231,400 people in the community with the virus, or around one in every 280 people in the UK is infected.&lt;/p&gt;&lt;/div&gt;&lt;/div&gt;&lt;div data-component=”text-block” e5tfeyi0″&gt;Vaccination of Scotland’s healthcare workers has lowered the rate of infection for people they live with, &lt;/b&gt;&lt;a href=”https://github.com/ChronicDiseaseEpi/hcw/blob/master/vaccine_manuscript.pdf” class=”ssrcss-hiczm3-InlineLink e1no5rhv0″&gt;according to a major new study.&lt;/a&gt;&lt;/p&gt;&lt;/div&gt;&lt;/div&gt;&lt;div data-component=”text-block” class=”ssrcss-uf6wea-RichTextComponentWrapper e1xue1i83″&gt;&lt;div class=”ssrcss-3z08n3-RichTextContainer e5tfeyi2″&gt;&lt;p&gt;The research looked at the households of more than 144,000 healthcare workers between 8 December and 3 March.&lt;/p&gt;&lt;/div&gt;&lt;/div&gt;&lt;div data-component=”text-block” class=”ssrcss-uf6wea-RichTextComponentWrapper e1xue1i83″&gt;&lt;div class=”ssrcss-3z08n3-RichTextContainer e5tfeyi2″&gt;&lt;p&gt;It found that the rate of infection for people that lived with healthcare workers was at least 30% lower when the worker had received a Covid dose.&lt;/p&gt;&lt;/div&gt;&lt;/div&gt;&lt;div data-component=”text-block” class=”ssrcss-uf6wea-RichTextComponentWrapper e1xue1i83″&gt;&lt;div class=”ssrcss-3z08n3-RichTextContainer e5tfeyi2″&gt;&lt;p&gt;The study said the “true” effect of vaccination could be even higher.&lt;/p&gt;&lt;/div&gt;&lt;/div&gt;&lt;div data-component=”text-block” class=”ssrcss-uf6wea-RichTextComponentWrapper e1xue1i83″&gt;&lt;div class=”ssrcss-3z08n3-RichTextContainer e5tfeyi2″&gt;&lt;p&gt;This was because people within the household could catch Covid from many different sources so the 30% relative risk reduction was an underestimate.&lt;/p&gt;&lt;/div&gt;&lt;/div&gt;&lt;div data-component=”text-block” class=”ssrcss-uf6wea-RichTextComponentWrapper e1xue1i83″&gt;&lt;div class=”ssrcss-3z08n3-RichTextContainer e5tfeyi2″&gt;&lt;p&gt;The research was led by Public Health Scotland and the University of Glasgow, and is said to be the first direct evidence that vaccinating individuals working in high-exposure settings reduces the risk to their close contacts.&lt;/p&gt;&lt;/div&gt;&lt;/div&gt;&lt;div data-component=”unordered-list-block” class=”ssrcss-uf6wea-RichTextComponentWrapper e1xue1i83″&gt;&lt;div class=”ssrcss-3z08n3-RichTextContainer e5tfeyi2″&gt;&lt;div e1xue1i83″&gt;&lt;div class=”ssrcss-3z08n3-RichTextContainer e5tfeyi2″&gt;&lt;p&gt;The study said that most healthcare workers in the research were women (79%) and the majority of the household contacts were men.&lt;/p&gt;&lt;/div&gt;&lt;/div&gt;&lt;div data-component=”text-block” class=”ssrcss-uf6wea-RichTextComponentWrapper e1xue1i83″&gt;&lt;div class=”ssrcss-3z08n3-RichTextContainer e5tfeyi2″&gt;&lt;p&gt;More than half of the healthcare workers were in “patient-facing” roles.&lt;/p&gt;&lt;/div&gt;&lt;/div&gt;&lt;div data-component=”text-block” class=”ssrcss-uf6wea-RichTextComponentWrapper e1xue1i83″&gt;&lt;div class=”ssrcss-3z08n3-RichTextContainer e5tfeyi2″&gt;&lt;p&gt;The study tracked the workers from 8 December and noted when they had their vaccine.&lt;/p&gt;&lt;/div&gt;&lt;/div&gt;&lt;div data-component=”text-block” class=”ssrcss-uf6wea-RichTextComponentWrapper e1xue1i83″&gt;&lt;div class=”ssrcss-3z08n3-RichTextContainer e5tfeyi2″&gt;&lt;p&gt;The majority received a first dose in December and January but 20% remained unvaccinated, the study said.&lt;/p&gt;&lt;/div&gt;&lt;/div&gt;&lt;div data-component=”text-block” class=”ssrcss-uf6wea-RichTextComponentWrapper e1xue1i83″&gt;&lt;div class=”ssrcss-3z08n3-RichTextContainer e5tfeyi2″&gt;&lt;p&gt;It found that there were 3,123 documented cases of Covid among their household contacts with 175 hospital admissions.&lt;/p&gt;&lt;/div&gt;&lt;/div&gt;&lt;div data-component=”crosshead-block” class=”ssrcss-mysbf6-ComponentWrapper-CrossheadComponentWrapper e1xue1i84″&gt;&lt;h2 class=”ssrcss-qozapo-StyledHeading e1fj1fc10″&gt;’Very encouraging’&lt;/h2&gt;&lt;/div&gt;&lt;div data-component=”text-block” class=”ssrcss-uf6wea-RichTextComponentWrapper e1xue1i83″&gt;&lt;div class=”ssrcss-3z08n3-RichTextContainer e5tfeyi2″&gt;&lt;p&gt;Statistical modelling showed that the rate of transmission was 30% lower for contacts of people who had received a vaccine dose at least 14 days previously. &lt;/p&gt;&lt;/div&gt;&lt;/div&gt;&lt;div data-component=”text-block” class=”ssrcss-uf6wea-RichTextComponentWrapper e1xue1i83″&gt;&lt;div class=”ssrcss-3z08n3-RichTextContainer e5tfeyi2″&gt;&lt;p&gt;It also showed that where healthcare workers had received a second dose of the vaccine at least 14 days before, the people they lived with had a rate of Covid-19 which was at least 54% lower than others who were close contacts of healthcare workers who had not been vaccinated.&lt;/p&gt;&lt;/div&gt;&lt;/div&gt;&lt;div data-component=”text-block” class=”ssrcss-uf6wea-RichTextComponentWrapper e1xue1i83″&gt;&lt;div class=”ssrcss-3z08n3-RichTextContainer e5tfeyi2″&gt;&lt;p&gt;Dr Diane Stockton, from Public Health Scotland’s Covid-19 Vaccination Surveillance Programme, said the results were “very encouraging” because they suggested that the vaccine helped prevent people from passing on the virus to others.&lt;/p&gt;&lt;/div&gt;&lt;/div&gt;&lt;div data-component=”text-block” class=”ssrcss-uf6wea-RichTextComponentWrapper e1xue1i83″&gt;&lt;div class=”ssrcss-3z08n3-RichTextContainer e5tfeyi2″&gt;&lt;p&gt;Leading epidemiologist Prof Keith Neal said the study underestimated the degree of protection.&lt;/p&gt;&lt;/div&gt;&lt;/div&gt;&lt;div data-component=”text-block” class=”ssrcss-uf6wea-RichTextComponentWrapper e1xue1i83″&gt;&lt;div class=”ssrcss-3z08n3-RichTextContainer e5tfeyi2″&gt;&lt;p&gt;He said household contacts of vaccinated workers could acquire infection from other sources.&lt;/p&gt;&lt;/div&gt;&lt;/div&gt;&lt;div data-component=”text-block” class=”ssrcss-uf6wea-RichTextComponentWrapper e1xue1i83″&gt;&lt;div class=”ssrcss-3z08n3-RichTextContainer e5tfeyi2″&gt;&lt;p&gt;This would mean that some of the Covid cases in this group could not have been prevented by vaccinating a household member who was a
